To place an order, please download and fill out the Order Form and send it in an email to cards4kidneys@gmail.com. You can also order by phone at 801-792-8213.
Once your order is received, an invoice will be sent to your email address, and you may then pay for your order via PayPal, check or cash.
Please remember that ALL proceeds from the sale of the cards will go directly to Paul Gibbs, so feel free to order as many packs as you would like.
